I can't say that I've seen anything like this, but one suggestion you didn't mention would be to replace her mail template. Try replacing it to some OTHER template and then back to the mail template. I would be sure to disable the OOA before doing so, and I think I'd even delete the OOA agent from her mailfile just to be absolutely sure it is created brand new when you apply the mail template again.
