Even if you had a caching issue, Notes cannot cache information from a database that never contained the information to begin with. You said you created a new database from scratch and -- I assume -- you copied over the subform that you had already modified. The old design never existed in the new database, so the Notes client had no chance to cache that information in the new database. Moreover, you deleted cache.ndk, which is where the cache is stored. What you're suggesting is logically impossible.
Therefore, your problem is not cache. You must look elsewhere.
Is it possible that the code you're seeing in the debugger is on the FORM rather than the subform? Maybe you have code in the same event on the subform also, but as you debug, you get into the form event code first.
Also, since you talked about creating new documents with the form, I'm assuming your problem is not existing documents with stored forms.
Do a design synopsis, or use TeamStudio Analyzer or configurator, and search to see where is the old code. It's in your design somewhere.
