Dan is absolutely right. No attachment content gets sent with the original email (only the attached file names and sizes). User actually have to select the "Open Attachment" option from the message menu in order to request the attachment. And then also the attachment only gets send in several parts depending on the attachment size (the converted attachment will be send in 2-3k chunks, I think), meaning that your users only have to pay for the parts of the attachment which they 'read' (or rather request by scrolling through the attachment).
Also if you are worried about your users requesting too large attachments for attachment viewing on the handheld then you could limit the supported attachment size to a maximum size that you think is reasonable. You can do that in the BlackBerry Server Configution icon on the Attachment Server's tab, check the drop-down box and select Attachment Server; in the table at the bottom you can specify a maximum size for each supported attachment type.
