Just throwing this one out there for reference and interest.
We have a large workflow database that controls a large amount of production. It uses response documents as a form of audit control (prior versions become responses), which means that we sometimes get replication and save conflicts.
I was asked to look at one Event that was missing its header document. All that could be seen in the standard views was a pair of save conflicts. Tracing the audit history through the $REF fields, I found a pair of non-conflict documents as follows:
Doc ID, $Ref value
Somehow, each one was a response to the other!
Using the child document I was able to find the correct one and removed its $Ref item, but I'm baffled as to how this happened.
