I have this document

In designer, that form is set "On Web Access" = HTML
and I can't change it for another problem.
I must insert a field where the web user can subscribe to newsletter. He should just insert his email and maybe his region and click "submit"
I could get this information by email or by creating a new form in a new database
Please Please Please help me!!
