Look further at the field / item

Ok, so we know there's something going on with that item on the document.

By inspect I mean check the properties in the debugger variables tab for that item within the doc object.

Could the field name possibly have a trailing space or something odd like that? So it would appear to the naked eye to be OK but not an exact match?

Try writing a quick agent to loop through the values in doc.items and compare item.Name to trim(item.Name) to see if they match?
