More Detail

Bummer, no takers.. Is there any more detail I could provide to assist anyone in assisting me? haha. When I open the form up after saving it, it shows two upload controls next to each other. But they are exactly the same and if I delete one, they both go away. Like this:

If I delete it altogether and add a new one from scratch it doesn't show up on the web page at all. Maybe it's just a corrupt form or something.
